About the Role

Apollo XCM LLC is seeking a highly skilled Patient Care Coordinator to lead our patient coordination efforts in Bogota, Colombia. As a key member of our team, you will be responsible for providing exceptional patient care by overseeing treatment plans and facilitating seamless communication between patients, caregivers, and healthcare providers.

Key Responsibilities
  • Lead Patient Coordination: Serve as the primary point of contact for patients, providing empathetic and efficient support. Address patient inquiries, manage appointments, and coordinate follow-up care to ensure timely access to medical services.
  • Collaborate with Teams: Work closely with interdisciplinary teams to streamline patient care coordination and improve overall outcomes.
  • Educate and Empower: Guide patients and caregivers through treatment plans, medications, and self-care strategies, enhancing health literacy and empowering them to take charge of their health.
  • Maintain Standards: Ensure accurate documentation of patient interactions, care plans, and progress notes in compliance with regulatory standards.
  • Advocate for Patients: Identify and overcome barriers to care, advocating for patient needs and connecting them with supportive services and community resources.
Requirements
  • Registered Nurse (RN) or MD license (US or Colombia).
  • Experience in healthcare coordination, patient advocacy, or customer service.
  • Bilingual proficiency (Spanish and English), with a required English level of C1 or above.
  • Strong multitasking and prioritization skills in a fast-paced environment.
  • In-depth knowledge of medical terminology and healthcare systems.
  • A compassionate, patient-centered approach.
  • Attention to detail and proactive problem-solving.
  • Familiarity with startup culture.
