Patient Care Coordinator Specialist

Apollo XCM LLC is seeking a highly skilled Patient Care Coordinator to join our team in Bogota, Colombia. As a key member of our healthcare coordination team, you will play a vital role in ensuring exceptional patient care by overseeing treatment plans and facilitating seamless communication between patients, caregivers, and healthcare providers.
Key Responsibilities- Lead Patient Coordination: Serve as the primary point of contact for patients, providing empathetic and efficient support. Address patient inquiries, manage appointments, and coordinate follow-up care to ensure timely access to medical services.
- Collaborate with Teams: Work closely with interdisciplinary teams to streamline patient care coordination and improve overall outcomes.
- Educate and Empower: Guide patients and caregivers through treatment plans, medications, and self-care strategies, enhancing health literacy and empowering them to take charge of their health.
- Maintain Standards: Ensure accurate documentation of patient interactions, care plans, and progress notes in compliance with regulatory standards.
- Advocate for Patients: Identify and overcome barriers to care, advocating for patient needs and connecting them with supportive services and community resources.
- Registered Nurse (RN) or MD license (US or Colombia).
- Experience in healthcare coordination, patient advocacy, or customer service.
- Bilingual proficiency (Spanish and English), with a required English level of C1 or above.
- Strong multitasking and prioritization skills in a fast-paced environment.
- In-depth knowledge of medical terminology and healthcare systems.
- A compassionate, patient-centered approach.
- Attention to detail and proactive problem-solving.
- Familiarity with startup culture.
- Contractor agreement with our U.S. headquarters.
- Flexible working hours: Monday to Friday, 8 a.m. to 5 p.m (CST).
