The Case Manager role is a telecommuter position that supports members with medical, mental health, and substance use challenges. What you will do Engage target members and build a relationship that supports open dialogue on sensitive topics. Conduct initial holistic health assessments to understand member needs, identify social determinants of health, and assess cultural influences. Collaborate with caregivers and providers to develop a member‑centric case management plan with goals, barriers, and outcomes. Monitor, evaluate, and update the plan to ensure progress toward goals and relevance. Facilitate communication and coordination among the health care team to maximize delivery of appropriate care. Provide interventions focused on removing barriers, promoting positive behavior change using motivational interviewing techniques. Maintain concise, accurate, and timely documentation that meets accreditation, contractual, and legal requirements. What you will bring – Required Active WA State or Compact license: Nursing (RN, ARNP), Behavioral Health (LMFT, LICSW, LMHC), or Psychology (board certification). At least 3 years of direct patient/clinical care experience post‑licensure. Reliable high‑speed internet (≥20 Mbps). Satellite and wireless connections are not supported. Preferred 1+ year experience as a Case Manager. Certified Case Manager (CCM) certification. Chemical Dependency Professional (CDP) or Chronic Care Professional (CCP). Lean training and advanced motivational interviewing skills. Licensing and Certification Expectations Case Managers must hold a valid WA state or MSL/Compact license and meet WA credentialing requirements. Non‑certified managers have 24 months to obtain a Certified Case Manager (CCM) credential. Training and Perks Initial and ongoing motivational interviewing skill development. Support to earn Chronic Care Professional (CCP) and Integrated Case Manager (ICM) certificates. NCQA accredited program with full credit for case management. LEAN methodology involvement in process improvement. Paid licensure renewal and CCM exam fee reimbursement. Paid membership in the National Case Management Society of America (CMSA) and local state chapters. Work Location and Schedule Telecommuter. Training: First 4–6 weeks – Monday through Friday, 8 am–5 pm PT. Ongoing: 40‑hour week with 5 hours for extended coverage (Mon‑Fri 5‑7 pm; Sat 9 am‑2 pm PT).
