Clinical Case Manager

The Clinical Case Manager plays a crucial role in providing alcohol, drug, and mental health treatment to patients in the Adult Outpatient Clinics. This position involves evaluating the mental health needs of patients, providing group counseling and individual therapy as needed, and coordinating case management services to further treatment goals.
Key Responsibilities- Identify dually diagnosed patients and coordinate effective treatment interventions.
- Determine when a patient needs to be referred for psychiatric evaluation and coordinate appointment schedules for the psychiatric team.
- Provide counseling, including group or individual interventions as necessary.
- Maintain current and accurate records as required by policy.
- Collaborate with external stakeholders, such as the Department of Human Services and Criminal Justice systems.

QualificationsTo be successful in this role, you will need a master's degree in human services, psychology, counseling, social work, or a directly related field from an accredited institution. You will also need two years of professional experience working with dual diagnosed patients and a valid registration in good standing with the Colorado Department of Regulatory Agencies (DORA) as a Licensed Professional Counselor (LPC), Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
