**Job description**

**Overview**:
We are looking for a proactive, organized, and empathetic **Coordinator (PSC)** to join our growing team. This role is essential to ensuring high-quality, timely care for our patients through efficient caregiver scheduling and exceptional communication with clients and their families. The Coordinator serves as a vital link between caregivers, patients, and the clinical team to ensure seamless care coordination and service satisfaction.

**Key Responsibilities**:

- Schedule and assign caregivers for patients in homes or facilities based on patient needs and staff availability.
- Initiate the intake process by creating a comprehensive patient profile to ensure the best caregiver matches.
- Use platforms such as **HHAexchange** and/or **Salesforce** to confirm visits and maintain accurate scheduling records.
- Communicate clearly with caregivers, patients, and families regarding assignments, updates, and any scheduling changes.
- Track and document caregiver availability, eligibility, and assignment acceptance.
- Document all communications and maintain accurate records in the system.
- Notify the clinical team of changes in patient condition or issues requiring clinical support.
- Participate in recruitment support by communicating staffing needs to the HR team.
- Ensure compliance with all internal policies and federal, state, and local regulations.
- Act as a liaison with contract case managers and schedulers, reporting any changes, issues, or client concerns.
- Provide on-call coverage when required, ensuring after-hours calls are answered promptly and resolved appropriately.
- Contribute to staff satisfaction and education through meetings, in-services, and training initiatives.
- Maintain strict confidentiality with patient and employee records and internal operations.

**Required Skills & Qualifications**:

- **Proficiency in English and Spanish** (spoken and written).
- Strong communication and customer service skills.
- High attention to detail and organizational abilities in a fast-paced environment.
- Knowledge of care coordination or scheduling in a home health or clinical setting is preferred.
- Proficient in Microsoft Office (Excel, Outlook, Word).
- Experience with **HHAexchange**, **Salesforce**, or other scheduling/CRM platforms is a plus.
- Ability to manage multiple tasks while maintaining professionalism and empathy.
- Willingness to work on-call shifts, including nights and weekends, if required.
