**Experiência**:Debe tener experiência en proyectos con población vulnerable en ONG, coordinando proyectos de cooperación internacional.

**Tipo de Contrato**:Prestación de servicios profesionales

**Responsabilidad del cargo**:
Establecer planes individuales para cada migrante y refugiado de Venezuela, lograr su vinculación, atención y tratamiento integral en salud.

Acompañar a los migrantes y refugiados de Venezuela en su proceso de vinculación a atención y tratamiento integral en salud.

Apoyar en los procesos de adherencia al tratamiento antirretroviral de migrantes y refugiados de Venezuela

Hacer entrega trimestral de tratamiento antirretroviral para aquellos beneficiarios que así lo requieran.

Hacer entrega trimestral de kits preventivos a los usuarios de los programas de AFA.

Hacer seguimiento para que los beneficiarios se realicen las pruebas de CD4, carga viral y demás exámenes de laboratorio complementarios, iniciales y control.

Gestionar la referencia y contrarreferencia de beneficiarios del proyecto para que hagan usos de los servicios disponibles en las rutas de atención locales de acuerdo con la necesidad de cada uno.

Realizar mapeo y focalización del territorio

Hacer seguimiento de los procesos de identificación de casos índices para el aumento de la positivización de casos en población migrante y refugiada.

Participar en la realización del informe de mapeo y focalización de la población objetivo.

Elaborar informe mensual de actividades realizadas de acuerdo con el formato definido representar al territorio ante entidades de gobierno.

Sueldo: $2.750.000 al mes
